Filling out forms before you get to your appointment can save time.

We are transitioning to electronic paperwork. You will receive a notification to complete your forms online before your appointment. You will also have the opportunity to fill them out electronically when you arrive for your exam using either an ARA-supplied iPad or your own device.

Please note: Mammography paperwork cannot be filled out ahead of time and must be completed at the imaging center. Also, certain exams may require that you fill out additional paperwork at your appointment.
